Doctor Referral

A prosthodontic consultation with Dr. Yepez may consist in multiple visits depending on the complexity of the case. In the 1st appointment Dr. Yepez will gather all the information necessary for an accurate diagnosis and adequate treatment plan. In the 1st visit Dr. Yepez will do an extensive dental examination, dental records and intraoral photography.

Please assist us by providing the following information at the time of your consultation:

• X-rays (if available)
• A list of your current medications
• If you have medical or dental insurance, bring the necessary policy information with you. This will save time and allow us to help submit any claims on your behalf.
