After payment of an annual fee, members will receive the following:

2 Regular Dental Check Ups:

• Comprehensive & Annual Exam includes: Periodontal Evaluation, Oral Cancer & TMJ Screening
• Regular Dental Cleaning & Oral Hygiene Instructions
• Digital X-Rays (Full Mouth Series 1X)

10% Discount off our fees:

• A treatment plan will be provided in writing with the discounted rates.

Prices:

• Adult: $350/year
• Couple: $600/year
• Child: $250/year

Local Business Owner:

• $325/year

Coverage goes into effect for 12 full months from the date of annual benefit payment.
